Does Your Current Insurance Cover Treatment in China?
Standard travel insurance: No. Basic travel insurance policies cover emergencies — lost luggage, trip cancellation, maybe an ER visit if you fall and break your arm. They do not cover planned medical treatment like IVF, LASIK, or knee replacement.
Domestic health insurance (US/UK/AU plans): Almost never. US Medicare, UK NHS, and most domestic insurance plans do not cover medical treatment outside your home country. There are rare exceptions (some EU plans have limited international coverage), but assume your domestic insurance won't work.
International health insurance: This is where it gets promising. Providers like Allianz Care, Cigna Global, AXA, Bupa, and Now Health offer international health insurance plans that cover treatment in China. Some of these plans even include direct billing with top Chinese hospitals.

Direct Billing: Which Hospitals Offer It?
This is the key question, because "direct billing" means the hospital bills your insurance directly — you don't have to pay upfront and claim reimbursement later.
Hospitals that offer direct billing:

Hospitals that do NOT offer direct billing:
Most public hospitals — including top ones like PUMCH, West China Hospital, and Beijing Tongren — require international patients to pay upfront. You'll need to pay the bill yourself and then claim reimbursement from your insurance. This means you need to have the cash (or credit limit) available, which for major procedures can be significant.
How to Pay at Public Hospitals
If you choose a public hospital (which many international patients do for their lower prices and excellent medical care), here's how payment works:
- The hospital's international department gives you a written cost estimate before treatment begins
- You pay a deposit (typically 50–100% of the estimated cost) before treatment
- For multi-day procedures, you may need to top up the deposit as costs accrue
- At discharge, any overpayment is refunded
- You receive itemized bills and medical reports in English (at most international departments)
- You submit these to your insurance for reimbursement
Accepted payment methods: international credit cards (Visa, Mastercard, Amex), Alipay, WeChat Pay, and bank transfers. Cash is accepted but not practical for large amounts.
Medical Evacuation Insurance
For serious medical treatment — especially cancer treatment, major surgery, or organ transplantation — medical evacuation insurance is strongly recommended. In the rare event of a major complication, you may need to be transferred to a hospital in your home country or to a regional center with specific capabilities.
